Her right hip struck the concrete floor and the metal threshold of a doorway. The hip was broken and she was hospitalized.

An employee was operating a standup clamp truck, transporting a large box (washing machine) across a facility. As the truck passed a structural support beam, the corner of a clamp at the back struck the beam. The resulting jolt caused the employee to fall out of the truck, which continued to roll forward and struck the employee's foot. The employee suffered a partially amputated big toe.

Two employees were fixing a commercial washing machine. As the drum was being manually spun to bring a broken airline into view, the injured employee's left middle finger became caught in the pulley system resulting in amputation of the fingertip.

The employee was attempting to unload the washed egg flats when the employee's left middle finger was caught in a chain driven by a sprocket that moved the egg flats along the conveyor through the machine. The employee suffered a left middle finger amputation.
